12. Optical Corneal Transplantation

Optical Penetrating Keratoplasty (Optical PKP) differs fundamentally from therapeutic corneal transplantation.

In TPK, the primary goal is to remove the infection and preserve the eye.

In Optical PKP, the infection has already been eliminated, but a visual problem remains because of a dense corneal scar or opacity.

Acanthamoeba keratitis

Anti-Acanthamoeba treatment

Eradication of active infection

Resolution and stabilization of the eye

Persistent dense central corneal scar

Continued visual impairment

In this situation, optical corneal transplantation may be considered to improve vision.

Optical keratoplasty is generally avoided while active infection is still present. Controlling the infection and ensuring that the eye has become stable are important factors for surgical success and for reducing the risk of recurrent infection or graft failure.
